  • Education Plan: DUE FRIDAY, OCTOBER 17, 2025
    • This document outlines your learning assignments for the academic year in your field placement and must be completed and reviewed with you and your field instructor.
    • To download, click here to obtain a copy of the Education Plan
  • 1 Monthly Process Recording & Timesheet: DUE LAST FRIDAY OF EACH MONTH
    • One process recording is due by the last Friday of the month. Please submit just 1 process recording with your field instructor’s comments.
    • A monthly timesheet that is signed by you and your field instructor is also due by the last Friday of every month.
    • Please upload your process recording and monthly timesheet to SONIA.

  • Mid-Year Evaluation Form: DUE FRIDAY DECEMBER 12, 2025
    • The Mid-Year evaluation is completed both by the student and the field instructor.
    • It must be signed by the field instructor and student on SONIA
    • Download a copy of the field evaluation form here
    • Upon review, the field advisor will submit the final fall grade to Hunter Student Services.
